This spotlight is on Dr. Patricia Byers, MD, FACS. She is a Professor and Section Chief in Surgical Nutrition and Metabolic Surgery at the Ryder Trauma Center/Jackson Memorial Hospital in Miami, FL. Dr. Byers became interested in nutrition while working in trauma and critical care, where she saw the impact of nutrition support on this patient population. She credits her career development to mentors like Drs. Stanley Dudrick, Bruce Bistrian, and Miguel Echenique. Dr. Byers also discusses the importance of a multidisciplinary team in the care of patients receiving nutrition support, and how all members of the team are able to learn and grow from each other. She highlights the minimal nutrition training that surgical residents receive and yet surgical patients can benefit the most from nutrition support. In her institution, she makes sure that her surgical residents are exposed to their nutrition support team and gain experience in writing orders for TPN. Dr. Byers is an active member of ASPEN and contributes to various committees, including the Physician Engagement Committee. This podcast features Dr. Jamie Bering, MD, Senior Associate Consultant in the Division of Gastroenterology and Hepatology at the Mayo Clinic in Scottsdale, Arizona. Dr. Bering first became interested in nutrition as a GI fellow after she realized her specific interest in placing PEG tubes and working with those patients on their nutrition and hydration management post-procedure. She describes the importance that mentorship played in her career advancement and specialty in nutrition. She has been fortunate enough to have completed her training in the same institution, now working alongside her mentors. Dr. Bering describes gaps in the field of nutrition specific to conditions where the role of nutrition support has not yet been well defined. She is interested in researching these areas and developing an algorithm to best approach these patients and determine how nutrition support might improve their overall outcomes. This video features Dr. Nisha Mangalat, Associate Professor of Pediatrics in the Division of Gastroenterology at the St. Louis University School of Medicine and Medical Director for the Glennon Intestinal Rehabilitation and Feeding (GIRAF) Program at SSM Health Cardinal Glennon Children’s Hospital. The GIRAF program was formed around a multidisciplinary, collaborative approach to caring for complex pediatric patients with intestinal failure. The GIRAF team meets weekly to discuss patient cases and review updates provided by the patients’ parents/caregivers. This enables the team to make proactive, systematic adjustments to each patient’s nutrition therapy. Dr. Mangalat shares that the name “GIRAF” was coined by their registered dietitian while attending an ASPEN meeting. Dr. Mangalat holds nutrition education very close to her heart and notes several gaps in the current medical education curriculums. Dr. Mangalat is the director of the pediatric gastrointestinal fellowship program and offers some insight into how we can engage future physicians to get involved in nutrition. She advises young professionals to not be afraid to say “yes” when it relates to their career and to explore if they are curious. Whether an avenue has been explored or not, Dr. Mangalat encourages others to think, “Why not me?” when researching something they have questions about. This video features Dr. Justine M. Turner, MD, PhD, Professor of Pediatrics and Divisional Director of Gastroenterology and Nutrition at the University of Alberta in Canada. Dr. Turner’s interest in nutrition stemmed from her original interest in Gastroenterology. Looking back, she felt there was a lack of nutrition education in her medical training, which she only realized once she began to pursue her PhD in nutrition. She notes there are too few physicians with PhDs in nutrition. Dr. Turner spends 40% of her time on research and 35% on clinical practice. Currently, she runs a translational animal lab looking at growth factors for adaptation, TPN lipids, and enteral nutrition in short bowel syndrome. In her clinical practice, Dr. Turner deals mostly with Celiac disease and gluten free diet while her inpatient service looks after solid organ and intestinal transplant patients. She highlights that nutrition is integrated in everything that she does. She also shares her insights on the differences between the delivery of healthcare in Australia and Canada and how each were affected during the Covid-19 pandemic. She insists that there is no better home for her interests than ASPEN. It has provided her with incredible opportunities to collaborate, present her data, and receive recognition for her work. ASPEN is also a reliable place for her students to learn and grow. She has served on the ASPEN research committee and currently serves as the Chair of the Publication Education Committee. With an adequate supply of multivitamins now available in the U.S. market, it is important to ensure that your clinical practice for dosing multivitamins for your patients on parenteral nutrition (PN) meets the standard of care. This video honors Dr. Praveen S. Goday, MBBS, Director of Feeding and Nutrition Programs at Nationwide Children’s Hospital in Columbus, Ohio. He is also a Clinical Professor of Pediatrics at The Ohio State University. Dr. Goday’s journey to nutrition was unplanned. He was recruited by his mentor to run the nutrition program in Milwaukee, despite having had no nutrition training. Dr. Christina Valentine discusses her NIH funded research on DHA supplementation for lactating mothers of premature infants. They have shown that 1000 mg of DHA daily taken by the mother decreases preterm infant inflammation through human breast milk ingestion. This video honors Dr. Mark Corkins, Division Chief of Pediatric Gastroenterology at the University of Tennessee Health Science Center and Le Bonheur Children’s Hospital. Dr. Corkins said he was fortunate in that nutrition was a focus in every step of his training. In medical school, his biochemistry professor said that students didn’t get enough nutrition and was given an hour each week to teach it. He later found out this focus on nutrition was not the norm in medical training. Dr. Corkins works as an academic physician and loves to teach nutrition. He says there are still huge holes in nutrition knowledge, despite significant progress in recent years. There are still lots of questions that need answers. This video honors Dr. Maitreyi Raman, Associate Professor of Medicine at the Foothills Medical Centre in Calgary, Alberta in Canada. She is also the Medical Director of the Southern Alberta Home Enteral and Parenteral Nutrition Program. After completing her nutrition fellowship around 2004, she noticed that there were very few nutrition-focused clinician/scientists within the gastroenterology landscape. She adds that her ability to find a female mentor in this field inspired and influenced her career path. Over the past 10 years, her research focus has shifted from enteral and parenteral nutrition to evaluating and accessing the role of dietary therapies, dietary components, and malnutrition in IBD. She encourages clinician/scientists to integrate nutrition into gastroenterology research reaching beyond enteral and parenteral nutrition to liver disease, IBD and beyond. This video honors Dr. Ajay Jain, Professor of Pediatrics, Pharmacology, and Physiology at St. Louis University, St. Louis, MO. His journey to nutrition began in medical school where he cared for babies needing parenteral nutrition (PN). While PN sustained them, many had liver injuries that progressed as they stayed on PN and needed liver transplants. This motivated Dr. Jain to focus on novel strategies to mitigate injuries and side effects of PN patients. He credits ASPEN with building his research niche. His first grant was from the ASPEN Rhoads Research Foundation. ASPEN’s resources, like Malnutrition Awareness Week, also helped him grow from a clinical perspective. Dr. Jain advises junior faculty to never give up on their ideas. He feels that today’s relative value unit (RVU) healthcare model does not give clinicians time to do research. Institutions need to protect and provide clinicians with mentorship, time, and tools to do this important work.
