- 32 - The role of an Occupational Therapist in dementia support (part 1)
Laura Stasinowsky is an occupational therapist (OT) and dementia consultant who is passionate about supporting people to live well with dementia. Laura works within the Your Brain in Mind clinic in Brisbane. She supports people at any stage of their journey with dementia, working with both the individuals who are diagnosed with dementia and their families.
Coral and Laura talk about the role of the OT in improving quality of life for a person diagnosed with dementia. They discuss how to communicate with someone who has advanced dementia and how making small changes to non-verbal cues can improve outcomes.

As Dad’s end of life approached, we worked together as a family to keep Dad comfortable and safe. We had great support from Coral’s GP, Dad’s geriatrician, and Dad's amazing carer, Kath.
Those last days were an emotional rollercoaster but from them we have some very special memories. Dad kept surprising us, right up until he took his last breath.
This is our story of supporting Dad to die at home. We want you to know that how you choose to support a loved one as they approach their end of life is entirely your choice. Whatever you decide to do, it’s okay.

Dad had four strokes in 2017, his last year with us. He developed delirium after each stroke. Dad did not recover so well after the fourth stroke and his needs were high. We discuss what that looked like for us from a caring role, and what aged care programs we were able to access to support us to bring Dad home from hospital.
Our priority was Dad's quality of life. We were in the very fortunate position of having strong family support and some extra money that allowed us to meet Dad's care needs at home.

Today we chat about our own father’s diagnosis of dementia and how it came about.
Coral was aware that things were changing with Dad and something wasn’t quite right. She started the process of getting a geriatrician involved. Dad moved through diagnoses of depression to mild cognitive impairment to dementia.
Coral was pushed into explaining the diagnosis to Mum and Dad because the geriatrician decided not to tell them. This meant Coral had to take on the clinical role and tell Mum and Dad what was happening and what to expect. It changed how Mum reacted and put undue pressure on Coral.

The support plan is the document that is given after a person has been assessed by either the Regional Assessment Service (RAS) or the Aged Care Assessment Team (ACAT).
When a person is assigned their home care package (HCP), they sign up with a provider. The care plan is the document that is drawn up with the home care provider. This guides what services and support a person will receive from their HCP.
These two different plans are both important. They are used at different times for a person’s support at home. We chat about what they are and when they are used. You’ll learn about the information that is needed for each. This information affects the level of aged care support you receive.

The wait times to receive some home care packages (HCP) have tipped out to around 12 months. This has increased significantly in the past year, from about six months in September 2023.
This article described the frustration of the adult daughter in not only having to wait such a long time for her mum’s home care package (HCP) to be assigned, but her belief that the providers couldn’t offer services under the home care package anyway due to a lack of staff.
We discuss the important points from this article and address some of the confusion around accessing in-home help.

Every person who is receiving a home care package should have a care plan that reflects their needs and goals, and includes the kind of support that will enable the older person to fulfil those needs or goals.
The care plan should be completed with the involvement of the older person and their family member or carer, if indicated.

Elder abuse can take many forms; physical, financial, psychological and social harms. We speak mainly about financial abuse in this episode.
Nick and MaryAnn talk about identifying the “red flags” and how to address them. It’s a fine line between advocating for the elder and pushing too hard. Experience in this area is so important.
MaryAnn discusses who the main perpetrators are, what coercion looks like, and understanding capacity. If people have a will and an enduring power of attorney, they report lower rates of abuse.

If an older person has lost some function or independence after a hospital admission, they may be approved for the Transition Care Program (TCP). This is a program of up to 12 weeks of clinician-led support to help the older person regain their strength and wellbeing and safely transition back to living at home.
TCP is approved while the older person is still in hospital, not after they have been discharged. TCP can be delivered in a number of settings, with the aim that it is completed in the older person’s own home.
Michelle discusses who approves for TCP and how the process works. Then she gives a real scenario, our dad’s use of TCP when he was twice discharged from hospital after having two of his four strokes.

When an Aged Care Assessment Team (ACAT) assessor does an assessment of the older person, they look at the person holistically. They want to understand what services and support could help that person to continue to live in their own home.
We discuss what the assessor is looking for, why they ask certain questions and why they may look around the older person’s home as part of the assessment.

More about Janine:
Janine discovered her love for community nursing in the metropolitan Sydney area of Canterbury in Sydney. Developing her skills Janine commenced in the role of Aged Care Assessor in an Aged Care Assessment Team (ACAT). She has worked as a Clinical Nurse Specialist, Psychogeriatric Aged Care Nurse and Team Leader in ACAT and Rehabilitation teams in metropolitan Sydney including Auburn and Kogarah, and interstate teams in Canberra and Queensland. It is within these teams Janine discovered she had found her passion, helping older people to remain living at home, whilst assisting them to maintain as much choice and independence as possible.
Following a move to the ACT in 2006, and after a period of working within the ACAT team, Janine was a Clinical Nurse Consultant for the Department of Veteran Affairs, providing consultation to other nursing services regarding the nursing care of veterans and DVA entitled persons who were over 65 and considered to have high level, complex needs.
In 2016 Janine was the Subject Matter Expert, national assessor and content creator for the My Aged Care national assessment workforce training.
Following another move in 2019 to the beautiful Sunshine Coast in Queensland, (which Janine feels will definitely be her last) Janine was providing aged care assessment as an ANACC (Australian National Aged Care Classification) Assessor. This assessment was introduced to improve the funding for people living in residential care.
Janine thoroughly enjoys her role with Coral and Michelle at See Me Aged Care Navigators. She is able to use all of her experience and skills to empower and assist others to understand and negotiate the national aged care system.

People often tell us they have had their My Aged Care assessment. This shows they do not understand where they are in the system. My Aged Care asks a series of questions. This is screening. This is not an assessment. The screening determines where this referral goes.
Know the difference between screening and assessment. Know who is assessing you. Different assessment teams approve for different programs, and the funding varies for each program.
The process takes time, too, so consider contacting My Aged Care if you are providing significant support to an older loved one.

After Dad passed away in January 2018, Coral began to write, starting with her blog “Dementia, my dad and me.” This was her way of dealing with the grief of Dad’s passing. This documented Coral’s journey with Dad over his final 8-10 years, his slow decline in cognition, his eventual diagnosis of Alzheimer’s, his final year of life and his eventual death.
At work, Coral was being asked more and more by her allied health and nursing colleagues to help families to understand what support could be utilised to get their older loved ones home from hospital and keep them there.
It was at this point that Coral realised she could do so much more to educate people on Australia’s aged care system so they could get the help they needed at home. So her business was born.
We care about people getting the right outcomes for their older loved ones.

We want to educate people to get the support for their loved ones that we were able to get for our dad as his needs changed, his dementia progressed and he reached the end of his life.
