Vasculitis is an autoimmune disorder, characterized by inflammation of the blood vessels. This inflammation can cause a range of symptoms, depending on the affected blood vessels, and it can impact various organs and tissues in the body. In general, inflammation develops in blood vessel walls which cause swelling, narrowing of the lumen, and weakness of the structural wall. The exact cause of vasculitis is often unknown, and the severity can vary.

In this amazing study, Dutch researchers used muscle biopsies in long COVID patients suffering from post-exertional malaise. They found that the muscle phenotype had changed, resulting in lower exercise capacity. They also found mitochondrial function issues and local amyloid accumulation. Macrophages and CD3+ T cells were also infiltrated in the muscle tissues.

The clinical takeaways are to keep inflammation under control, utilize light exercises and not stressful ones, and optimize mitochondrial health.

In this in-vitro study, researchers demonstrate that a cardiac tissue infected with SARS-COV-2 can contain the viral genome for days after infection. Not only is the viral genome present, but viral reactivation occurs as soon as this cardiac tissue undergoes a hypoxic stress state. The researchers recommend that clinicians look for the presence of the virus, or its genome, or cardiac troponin levels, to monitor at risk hearts. Let's review.

In this review of research, we find that SARS-COV-2 (possibly directly) and COVID-19 vaccines (possibly indirectly, via anti-idiotype antibodies) can change the neuron gene expressions in the sensory ganglions. Additionally, there is a possibility of ganglion pathology due to peripheral nerve injury and systemic inflammatory molecules in the circulation. Satellite glial cells, macrophages, and other immune cells could become activated in the ganglions resulting in neuronal hyperexcitability and hyperesthesia and allodynia. Let's review.

In this study, researchers found that the genetic material from the virus and possibly other parts activate TLR3 receptors. These in turn cause the cells to release IFN1. IFN1 works on the intestinal cells via STAT1 pathway to down-regulate ACE2 receptors and B0AT1 channels. The result is reduced absorption of tryptophan leading to reduced serotonin production in the body. They also found that IFN1 activates platelets leading to micro thrombi and further reduction of serotonin as platelets are a huge reservoir of serotonin. Researchers also found that MAO activity was increased. Vagal activity was reduced leading to the hippocampal reduction in function as well. The researchers suggest using tryptophan precursor supplement (5HTP - 5 hydroxytryptophan) and SSRI medications to rescue serotonin levels in long COVID patients.

In this study from Israel, researchers found that sperm concentration reduces after Pfizer-BionTech vaccine, and then recovers after some months. Here are the results they posted with the study:

Repetitive measurements revealed −15.4% sperm concentration decrease on T2 (CI −25.5%–3.9%, p = 0.01) leading to total motile count 22.1% reduction (CI −35% – −6.6%, p = 0.007) compared to T0. Similarly, analysis of first semen sample only and samples' mean per donor resulted in concentration and total motile count (TMC) reductions on T2 compared to T0 - median decline of 12 million/ml and 31.2 million motile spermatozoa, respectively (p = 0.02 and 0.002 respectively) on first sample evaluation and median decline of 9.5 × 106 and 27.3 million motile spermatozoa (p = 0.004 and 0.003, respectively) on samples' mean examination. T3 evaluation demonstrated overall recovery without. Semen volume and sperm motility were not impaired.

In this fascinating new study (October, 2023) researchers from Germany demonstrate that mRNA COVID-19 vaccines cause a long-term change in the immune profile, resulting in the immune system adjusting its biomarkers to accommodate for these changes. And, those individuals that fail to accommodate for the long-term vaccine-related immune changes end up with vaccine injuries. In other words, their immune system gets vaccine-related modifications on top of their existing baseline and this combined result leads to injury.

So what needs to be measured? IL-6/IL-8 levels and antibodies for angiontension II type I receptors (ATR1) and alpha2b adrenergic receptor (alpha2b-adr-R).

The researchers say in the study: “We present a set of observations that are potentially relevant for the understanding and diagnosis of PACVS, a dysautonomia syndrome associated with, and possibly triggered by, SARS-CoV-2 mRNA vaccination [1]: In healthy persons not affected by PACVS, the repertoire of receptor antibodies involved in cardiovascular regulation and immune homeostasis undergoes long-term adjustment following SARS-CoV-2 mRNA vaccination. The above adjustment seems blunted, absent or even inversed in persons who present clinical phenotypes of PACVS after SARS-CoV-2 mRNA vaccination.
PACVS-afflicted persons can be distinguished from individuals subjected to SARS-CoV-2 mRNA vaccination without developing PACVS based on serum levels of IL-6/IL-8 and antibodies against AT1R and α2b-adr-R.”

MIS-C is also referred to as Pediatric Inflammatory Multisystem Syndrome temporally associated with SARS-COV2 (PIM-TS).

Common symptoms of MIS-C in babies show as a rash and/or diarrhea. In older children, the symptoms are respiratory.

MIS-C is similar to Kawasaki’s disease, although the symptoms usually resolve in contrast to Kawasaki’s disease.

Treatment for MIS-C includes:
• supportive care and observation
• maintaining volume status as dehydration worsens symptoms
• steroids
• IVIG
• Interleukin blockers
• anti-coagulation
• cardiology work-up with an EKG and cardiac echo

In this episode of ‘Kid's Corner’, pediatrician Dr. Liz Mumper explains the link between food behavior and polyphenols. Foods that contain polyphenols include blueberries, plums, cherries, apples, strawberries, black currants, black olives, dark chocolate, black tea, coffee, hazelnuts and pecans. Some spices, including turmeric, cloves, cinnamon, ginger and cumin are also high in polyphenols. When children have an intolerance to polyphenols, they can exhibit strange behaviors including hyperactivity, aggression, night sweats and sleep issues, and their ears and cheeks may become flushed. Dr. Mumper notes that this intolerance occurs more often in children with autism. She also discusses strategies for discovering if your child has this dietary issue.

In this study, researchers demonstrated that the use of metformin within the first four days of COVID-19 symptoms reduced healthcare utilization and the incidence of long COVID. The researchers write: "In conclusion, early outpatient COVID-19 treatment with metformin decreased the subsequent risk of long COVID by 41.3% during 10-month follow-up. This finding is consistent with the 42.3% reduction in healthcare utilization for severe COVID-19 with metformin in the first 14 days of the trial." Let's discuss.

In this early pandemic study of COVID-19 confirmed hospitalized patients, researchers found two groups of individuals with biomarker profiles: the first had high fibrinogen levels in relation to C-Reactive proteins (CRP) and the second had an increase in D-Dimers in relation to CRP. These molecules generally increase in relation to CRP; however, here the researchers found that the fibrinogen and D-Dimers increased while the CRP stayed low. These cohorts were associated with higher likelihood of neurological long COVID. Let's review both the findings and the mechanisms.

In this fascinating study from UCSD, researchers demonstrate for the first time that time-restricted-feeding (TRF), or intermittent fasting, can synchronize neuronal tissue clocks. This results in the Alzheimer's disease mice to have better sleep, improved sleep length, less irritability, better metabolic dynamics, reduced plaque load and improved plaque clearance, and improved abeta42/abeta40 ratio, with memory improvements almost equal to healthy mice. This amazing study demonstrates the influence of plant-based (high-fiber) and/or fermented food diets on our immune system.
