Tue, 29 Oct 2024 - 30min - 215 - Fasting Q&A with Dr Jason Fung: Sauerkraut, Stress and Body Set-Point, Enemas, Replenishing Glycogen After Exercise, Protein Requirements, and More
Episode #175
In this Fasting Q&A episode, hosted by Coach Lisa Chance, Dr. Jason Fung answers questions from the TFM Community:
- Is sauerkraut OK for fat fasting or olives only? [03:10] What happens to the body weight set point when we are under stress? [04:11] I recently listened to a podcast from a few years ago where you and Dr. Peter Attia discussed insulin resistance. You said you had evolved your thinking about it and that it was not like a lock and key that didn't fit, but, instead, more like an overflow situation. You described it as a suitcase (the cell) already filled to the point of overflowing with shirts (the glucose), and it just couldn't hold any more, you just couldn't stuff anymore in. You also said that it is hyperinsulinemia that drives metabolic syndrome, not insulin resistance. Is that your current view? [12:13] Does an enema stimulate insulin? For example, a coffee enema with kefir, whey, or other added probiotics. [16:00] When we exercise, our bodies will pump out blood glucose to help with the energy needed. For an insulin resistant person doing hard, aerobic exercise (such as HIIT or run/walk), does the raised blood glucose need insulin to process it? [17:31] When a person is doing regular exercise routines, do they need to replenish glycogen stores after a workout or does the body do this on its own? [19:22] I read an article recently about SGLT inhibitors which said they can stimulate autophagy. Is this true? [24:33] Protein requirements are so confusing. If a person has a lot of weight to lose and they have a lot of excess skin (which is protein you want the body to use to minimize all that loose skin when you are done losing weight), how do they know how much protein to eat? Would they need less protein the more weight they had to lose? Does eating less protein force the body to use the loose skin? Does eating too much or too little protein have consequences? [27:40] I have read some things that say 8 ounces of animal products per week is enough and more than that has negative effects (insulin resistance, elevated bad cholesterol, and other things), and so getting more protein from legumes, nuts, and seeds is a good idea. Also, the type of animal products makes a difference; it is not just the processing, but whether it is organic, pasture-raised, regenerative, etc. I think that it can be easy to interpret that, since saturated fat isn't as bad as we thought, we might eat more animal products or not-so-healthy animal products, and, while it might lower insulin and promote weight loss in the short term, there can be other long-term effects that are not so healthy. Can you comment? [34:19]

Tue, 15 Oct 2024 - 44min - 213 - Fasting Q&A with Megan Ramos: PMS, PCOS, Vitamin D, Dark Chocolate, ADHD, and More
Episode #173
In this Fasting Q&A episode, Megan Ramos answers some of your fasting questions:
- Why is it that, when I am consistent with fasting and diet, my PMS and period symptoms (cramps, headaches, and backache) are lessened? Is it to do with insulin? (Jenna from Michigan) [01:30] I keep hearing mixed answers about fasting for PCOS. Some say it’s really beneficial and then others tell me to eat 5-6 small meals a day and not to fast or have coffee on an empty stomach because of the cortisol response. Can you clarify please? I feel better when I fast. I’m still only fasting 14-18 hours per day and trying to stick to TRE but I’m afraid I’m making it worse with my cortisol response. Help! (Amy from South Dakota) [02:45] I’ve been trying to heal my PCOS with ADF for about six weeks now. I have started spotting occasionally and sometimes have shorter periods. Could this mean that my body is healing and trying to self-regulate and get my cycle back to normal? (Sarah from Maryland) [08:12] I take vitamin D daily because my levels were low a few years ago. They have since returned to normal. Since vitamin D is a fat-soluble vitamin, do I have to eat something with it on my fasting days or can I take it without and still get the benefit? (Beth from Nova Scotia) [09:47] Should you eat dark chocolate every day? I read a study that says, “The research findings suggest that eating 48g of 70% dark chocolate each day may help lower fasting glucose levels and reduce insulin resistance.” I’ve also heard that dark chocolate reduces LDL cholesterol. Do you agree with this? (Michelle from British Columbia) [12:17] I need some mindset help! I am 53 and spent most of my 40s gaining weight. I spent hours and hours every day unhappily thinking about my weight. My doctor told me about TFM and, after four months of TRE and 24,42, 48-hour fasting, I reached my goal weight. I was looking forward to the mind freedom but it’s been nine months now and I am still consumed by thoughts about weight and fasting. I eat whole foods, little sugar and processed foods, and fast 24-48 hours three times a week with TRE the rest of the time. I just want the mental freedom that so many people talk about. Help! [14:46] I’m a 40-year-old female and have lost 53 pounds following different protocols but mostly intervals of OMADs 3-4 times a week and 2MADS for nearly two years. Recently, I’ve also added one 42-hour fast a week. I’ve noticed that I’m just not hungry. I end up eating based on the planned windows which are scheduled around workouts and a busy schedule. Should I wait for hunger and so extend my fasts further? I feel more like I *want* to eat than *need* to eat. (Michal from Israel) [18:25] I keep hearing that fasting is different for men and women and that women can actually gain weight if we fast at the wrong times. I am a 48-year-old woman. Is this accurate? [20:33] Do you have any tips for fasting when you have ADHD, specifically, the lack of dopamine and how food can be a source of dopamine. What other ways can we get dopamine, especially in the immediate and easy way eating provides? [22:39]

Tue, 01 Oct 2024 - 36min - 211 - Fasting Q&A with Dr Jason Fung: Gallstones and Fasting, High Ketones, Persistent High Blood Sugar, and More
Episode #171
In this Fasting Q&A episode, hosted by Coach Lisa Chance, Dr. Jason Fung answers questions from the TFM Community:
1. Imagine that I dress the carbs that I eat by adding acid and fat so that my BG spike is minimized. What happens to all the fat that I added? Don’t the carbs send the signal for insulin to go up and, consequently, for all the fat I added go directly into body fat? If so, should I minimize the amount of fat that I dress carbs with and mostly just eat them with acid, like vinegar or lemon juice? [01:55]
2. What eating strategy would you suggest for an insulin-resistant or pre-diabetic person? In other words, do they need to low carb to get out of the pre-diabetic range? [08:32]
3. I am struggling with getting my blood sugars down to a normal range and am stalled in my weight loss. I have lost 150 lbs and gotten off insulin and other oral meds. I feel like my way of eating is on point 95% of the time and I am fasting 24-48 hours or longer 2-3 times a week. On my last extended fast, my blood sugar didn’t come down until day 3 and then started going up every day. By day 5, my blood sugar was 189. I broke my fast with whole, low-carb foods and my blood sugar came down to the 140s where it likes to live these days. I'm wondering why, so far into my journey, things would start going wonky. I do have a lot of personal and family stress happening right now. Could that be generating enough cortisol to be negating my diet and fasting or are there other explanations that could be contributing? [16:44]
4. Do you have any evidence (empirical or otherwise) on fasting, autophagy, and reduced carbohydrate intake for people with ADPKD (Autosomal Dominant Polycystic Kidney Disease)? [20:23]
5. Can patients with gallstones fast? Does it increase the risk of gallstones? [24:57]
6. I know seed oils are not recommended but what do you think about cold pressed niger seed oil? Avocado oil is not readily available in my country, and I cannot use butter or animal fat everyday as there are a minimum of 180 religious fasts a year. These fasts prohibit dairy products and meat, so we are basically vegans during these fasts. [30:36]
7. Why do some people (who have never missed a meal in their life and suddenly start a multi-day fast) get very high ketones and have to break their fast? [34:32]

Tue, 03 Sep 2024 - 21min - 206 - Fasting Q&A with Dr Jason Fung: Stomach Rumbles, ‘Anchoring’ Weight, Stress and Insulin, Body Set Weight, and More.
Episode #167
In this Fasting Q&A episode, hosted by Coach Lisa Chance, Dr. Jason Fung answers questions from the TFM Community:
- If the stomach rumbles about an hour after having a black coffee, does this mean that the coffee raised insulin? [01:43] Is insulin released only when the food is in the mouth? It feels like when a big, heavy meal is in the stomach for hours I could still add a supplement or snack to it and it wouldn’t make a difference, but would there be an additional insulin release? [02:54] I have read that it is recommended to ‘anchor’ your weight after a 10% reduction in BF%. Can you say a bit more about this? I presume you would ‘anchor' by scaling back on fasting, and adjusting frequency and intensity in response to any changes in BF% so as to keep it stable. But what happens if you go all the way in one go? Do you need to ‘anchor' for longer at the lower BF% in order for it to become your new set weight or does the research say it’s not possible to reset it that low if you didn’t pause along the way? [03:35] Is it true that a longer fast (or intense exercise) can stress your body, causing insulin to go up, and, therefore, might even be counterproductive? [14:08] One of the biggest effects of blood glucose and insulin spikes is the damage to small blood vessels. These spikes damage the organs that predominantly rely on small vasculature like kidneys, eyes, nerves in fingers and toes, leading to complications like diabetic kidney failure, macular degeneration, and neuropathy. Hyperinsulinemia is really a microvascular disease. Could you please share how you explain this process to non-medical folks? I fail to explain to my family members in layperson terms exactly what it is about the spikes of BG and insulin that damages the small blood vessels. [16:24] In The Obesity Code, you wrote about a study where they overfed people to get them to gain weight. The study showed that they had a hard time putting weight on because their bodies knew to burn it off. So my question is, if our bodies experience this effect as per the study, what signs would there be (such as raised temperature and/or heart rate) to get our body to burn the extra calories consumed? [22:57]

Tue, 27 Aug 2024 - 34min - 205 - Hot Topic: Therapeutic Fasting vs Fasting Sometimes (REPLAY)
SUMMER BREAK As Terri takes a couple of weeks away from her podcast duties, please enjoy a replay of one of the most popular episodes.
Episode #166 (#137)
In this episode Nadia and Terri dive deep into why so many of us struggle to make progress and therefore stay stuck in the uncomfortable space of starting/stopping, or how many of us push ourselves so hard that we can't sustain consistently to see the lasting results.
Tune in to find out how our brain can sometimes trip us up when it comes to motivation because of its love for novelty, and why so many people start the New Year with good intentions but struggle to maintain that momentum for long enough to see lasting results.
How do we change our mindset to overcome these issues? Terri shares some great analogies to help adjust our mindset and behaviour and take confident steps towards the success we have been looking for, and Nadia reinforces the importance of taking a step-by-step approach towards therapeutic fasting that starts with, of course, TRE (time-restricted eating).

Tue, 20 Aug 2024 - 40min - 204 - Expert Guest: Coach Lisa Chance (REPLAY)
SUMMER BREAK
As Terri takes a couple of weeks away from her podcast duties and after Coach Lisa's episode last week about cortisol-lowering techniques, it seemed like a good time to remind podcast listeners about Coach Lisa's story.
Episode #165 (#35)
In today's episode, Megan and Nadia interview their colleague, Coach Lisa Chance, a retired RN. Megan and Nadia chat to Lisa about how she started fasting and all of her successes over the last 4 years. Listen in to hear Lisa talk about:
- how TRE (time-restricted eating) is the foundation of her journey
- how she lost over 160 lbs in total, but, more importantly, how she has kept it off
- how she went from a size 26 to a size 10 and her waist is down 22 inches
- how she reversed SO MANY health concerns that she created a list of NSV (non-scale victories) for our community to look forward to
Lastly, listen in as Coach Lisa (in the most touching of ways) tells us how she started working for The Fasting Method program and all the amazing things she does in our community.

Tue, 06 Aug 2024 - 34min - 202 - Fasting Q&A with Dr Jason Fung: Order of Fat Burning, Fasting With a Gastric Sleeve, BMR Fluctuation, GLP-1 Drugs, and More
Episode #163
In this Fasting Q&A episode, hosted by Coach Lisa Chance, Dr. Jason Fung answers questions from the TFM Community: I have a Lumen device and they talk about a fasting ‘sweet spot’. They say that if you fast for too long, then your body might get stressed and go back into carb burning, and that if you test with the Lumen every two hours, you can see this switch from carb burning to fat burning and then possibly back to carb burning – in which case it could be a good idea to stop the fast, perhaps doing more shorter fasts to maximize fat burning. Is this true? [01:57]
- I have heard conflicting reports about lemon in water and herbal teas breaking your fast. Do they will interfere with autophagy, if that is your main goal? [08:47] Is it known if there is an order in which fat is burned during fasting? For example, is the recently-gained fat burned first and the oldest last, or visceral vs subcutaneous? [10:52] My doctor is not discouraging fasting but dismissed it as another means of cutting calories. How can I explain concisely why fasting works but calorie reduction diets don’t? [13:55] What are your thoughts on using GLP-1 drugs in conjunction with fasting? [21:25] Any suggestions for successful fasting protocols for individuals, post gastric sleeve (3 years)? [23:45] In his book The Obesity Code, Dr. Jason Fung explains that a large percentage of obesity is genetically determined. Could the outcome of the research also be explained by us getting addicted to sugar before we are born if our mother consumes sugar? [24:56] Since fish oil contains no protein and thus should not trigger mTOR, isn’t it the best fasting aid as it is both and-inflammatory and calming? [29:38] I know that in some studies in the past they have showed that not having an “up day” after a full fasting day in ADF would lead to a decrease in BMR. Do we still believe this to be true? If I did a 48-hour fast, ate one meal, and then fasted for 24 hours right after, and it wasn’t difficult, am I ultimately decreasing my BMR? [30:37]

Tue, 02 Jul 2024 - 39min - 197 - Fasting Q&A with Dr Jason Fung: Fasting With Low BMI, Oxalates, Metabolic Flexibility, Cancer Treatment and Fasting, Magnesium Supplementation, and More.
Episode #158
In this Fasting Q&A episode, hosted by Coach Lisa Chance, Dr. Jason Fung answers questions from the TFM Community:
- Does fasting help with not choking for an impacted oesophagus? [01:58] I have been struggling with my fasting due to terrible headaches that last days after 36-hour fasts. I've been put on a bunch of supplements to support me with detoxification including liver supplements, B vitamins, and activated charcoal, but I find that these can sometimes make the headaches worse. I've been advised to do shorter fasts but this doesn't result in any weight loss. Any idea how I can move past this? [03:36] How many meals between fasts is ideal for weight loss? Two or three meals a day? [07:21] I understand how excess carbohydrates can be stored as fat (via insulin)., but I'm having a hard time understanding how dietary fat is utilised and stored if we go low carb and increase our fat intake. Isn't dietary fat stored as fat? I am confused about how we are able to burn body fat while eating more dietary fat. [09:25] Which fasting protocols would you recommend for people who have a low BMI but still have insulin resistance? [13:09] Does miso soup break a fast? It would be a great source of salt and I prefer savory drinks to sour. [15:33] I got a CGM so that I could see my glucose response to various foods. Is it correct to assume/conclude that, if I see an increase or spike in glucose, there will be a corresponding increase in insulin? And, conversely, if I do not see an increase in glucose, there is little or no increase in insulin? Could there be a situation where glucose does not go up but insulin does? Is there a correlation between an increase in ketones and a decrease in insulin? [16:41] I’ve measured consistently high in oxalate on an OAT test over the last decade. I’ve read that oxalates are difficult to measure in the body, outside of stones which I'm hoping to preempt. Will fasting, detoxing, releasing toxins help reduce high oxalates? [18:03] Fasts longer than 48 hours have triggered inflammation in individual finger joints. Is that a potentially common way for detoxing or autophagy to present (meaning, the specific, targeted area rather than more broad based)? [19:59] Could you please address the subject of metabolic flexibility? Is it possible to be too fat adapted? [21:22] Since cancer is a disease of too much growth and fasting slows down growth, why is it not a good addition to *most* chemo therapies when the patient has sufficient fat mass to allow for fasting? [24:45] In their book, Brain Energy, Dr. Chris Palmer talks about how ketones are beneficial for brain function. In your Toronto clinic, back in the day, did the patients who fasted under your and Megan Ramos's supervision anecdotally report mental health improvements as they progressed through therapeutic fasting regimens? [30:42] Why do different people need potentially drastically different amounts of magnesium supplementation when they eat the same foods? My partner and I are almost identical age and weight, and have the same activity level and diet. They do not benefit at all from magnesium supplementation, while I need almost four times the recommended daily dose to feel good and not have muscle cramps. [32:21]

Tue, 04 Jun 2024 - 36min - 193 - Fasting Q&A with Dr Jason Fung: The Dawn Effect, Smart Scales, Kombucha, Cortisol/Stress, Nausea from Salt, Fat Adaptation, and More.
Episode #154
In this Fasting Q&A episode, hosted by Coach Lisa Chance, Dr. Jason Fung answers questions from the TFM Community:
- If someone is glucose sensitive, not insulin resistant, with an A1C of under 5.0, would they still have a dawn effect happen? If they did, what would the dawn effect bump in glucose possibly be? [02:12] I have a smart scale. In addition to weight, it has metabolic rate, body fat %, etc. I am interested in knowing if the relative changes (as opposed to the absolute number) would be useful to know. Similarly, I am interested in reducing visceral fat. If that number goes down by 10%, might that be a helpful indicator about the direction that things are going in and whether what I am doing is working? [05:56] What’s your take on kombucha? [09:35] I read a book named “The One Minute Cure: The Secret to Healing Virtually All Diseases.” It is a therapy with H2O2 in Food Grade of 35% or 3% (diluted in Distilled Water). I have tried this therapy and every time I test my blood sugar after 30 minutes of taking it, it lowers my blood sugar by more than 17 mg/dL. I was wondering if you have studied this therapy and what you think about it? [11:32] I find it easier to fast during work. However, I have a stressful job with long hours. Could cortisol/high stress be negating the benefits of the fast? Would I be better off using TRE and save fasting for my off days? [13:08] This month’s TFM Group Challenge is ‘Giving Up Your Vices’. Megan’s first video discussed A2 dairy. What is your take on this and what is the appropriate amount of A2 dairy each day? I know Coach Nadia says ‘sparingly’, but it’s not clear what that means when it comes to sheep/goat/A2 dairy. Does it matter what kind of dairy it is if it’s still all insulinogenic? [15:13] I’ve tried many different forms of salt and even tried making my own salt capsules, yet, without fail, I end up feeling nauseous shortly after taking it. Is there anything else I can do to quell the nausea? Do some people just not need salt or have a medical reason why salt makes them nauseous? Is there anything else I can try? [19:06] How high should a [blood sugar] spike be after meals, and when should your blood sugar levels return to the pre-meal range? [21:46] How do I know when I’m fat-adapted?[23:37] If someone has high triglycerides, is it a pretty good guess that they also have non-alcoholic fatty liver disease? [26:07] Do you have any tips on how I can do intermittent fasting without losing weight? My goal is to lower my A1C and be less dependent on medication while improving my health. [28:02] I am a lifetime vegetarian and have always eaten very clean, whole foods. I have been fasting since 1979 and I easily fast for weeks, change up the fasting protocol, etc, but the pounds just will not budge. How can I use fasting to drop this weight? [30:39]

Tue, 21 May 2024 - 42min - 191 - Fasting Q&A with Megan Ramos: Hair Thinning, Plateaus, Caffeine Sensitivity, Digestion Speed, and Keto Breath
Episode #152
In this Fasting Q&A episode, Megan Ramos answers some of your fasting questions:
1. I have just started fasting and am struggling to extend my fasts to 36 hours as I have two small children and we eat dinner together every day. Any tips? [01:57]
2. I do IF five days a week, a 36-hour fast and no fasting on the other two days. I feel fantastic and the weight is coming off, However, my hair has recently started thinning. I’ve heard this could be because fasting is a stressor on the body. I also work out 3-4 times a week and have two jobs, one of which is pretty stressful and high pressure. Is the weekly 36-hour fast just too much for me? [07:40]
3. I’ve progressed into a 3x42 protocol over three months and the NSVs and systemic healing has been miraculous! I’ve not seen a weight or body-fat decrease over the last four weeks after a 25-pound loss in the first few months. Is a plateau this long normal? [12:25]
4. Would you recommend leaning heavily into fat fasting when initially trying to establish a pattern of therapeutic fasting? I am finding it easier to do longer fasts (24 hours+) when I fat fast beforehand, but have some concern that maybe this could have long-term implications. It almost seems like ‘dieting’ which will end once the therapeutic level of fasting ends and then I will have to reestablish another eating pattern? [16:16]
5. I quit coffee a few months ago as it was affecting my sleep and I would also experience extreme sugar cravings 1-2 hours after drinking it. Since stopping coffee, I have noticed similar effects with green and black tea plus I also get the jitters and an increased heart rate. Most recently, I have been getting insomnia after eating dark chocolate. Clearly, I am becoming more sensitive to caffeine! Is this common for fasters? Or maybe it is age related? I’m a 43-year-old woman. [20:50]
6. I have been fasting for almost two years (alternate-day fasting). I notice it takes longer for me to digest meals now vs before I started fasting. Is this normal and does this mean it also takes me longer to get into a fasted state now? [24:59]
7. I need help with keto breath/tongue! I’ve tried drinking more water, mouth wash, tongue scrapers, food-grade hydrogen peroxide mixed with water. No change! I makes me gag so much I just want to eat to make it stop! The only thing I haven’t tried is oil pulling. Help! [28:51]

Tue, 30 Apr 2024 - 20min - 188 - Fasting Q&A with Dr Jason Fung: Broccolini, Metformin, Exercise and Weight Loss, and More
Episode #149
In this Fasting Q&A episode, hosted by Coach Lisa Chance, Dr. Jason Fung answers questions from the TFM Community:
- I’ve heard you’ve written an amazing book called, “The Cancer Code.” I haven’t read it yet, but wanted to hear your thoughts on broccolini being an anti-cancer food. I understand it contains Sulforaphane, Indole-3-carbinol (I3C), Glucosinolates, Vitamin C, and fiber. Should I concentrate on cruciferous vegetables, like broccolini, or just work on getting a balanced diet? What are your thoughts? [02:14] I have a little confusion about the difference between taking metformin and injecting insulin. From what I've read in your books it's that insulin is going to make type 2 diabetes worse in the long run. I already know that that’s not the answer to reverse type two diabetes and I’m not taking either one. Like insulin, will metformin also be detrimental to treat T2D or is there a difference between one and the other - and why, please? [05:27] What do they mean by 'immunoreactive insulin'? [12:06] Does the liver respond negatively to the ingestion of supplements? In other words, do the supplements cause any changes in my insulin? [14:52] I started TRE (beginning Jan this year) at about 135 pounds and am now doing 24-hour fasts with OMAD and 2MAD. I have completed a 42-hour fast and will probably try another one this week. I am eating whole foods. Avocados are my miracle!!! I do not snack between meals. My body is still releasing weight with what I am doing now and I feel I have another 5 pounds to go. How will I know when to stop shedding the weight? [18:13] Can you clarify the role of physical activity and exercise in weight loss? In The Obesity Code, you shared that a number of rigorous clinical trials demonstrate that exercise contributes minimally, if at all, to weight loss. However, zone 2 exercise is promoted for its'fat burnin' benefits. I plan to continue exercising for its health-promoting benefits but I would really like to hear your take on the extent to which it can assist with weight loss, and what exercise types you recommend for weight loss. [21:56] On non-fasting days, what is Dr. Fung’s chosen style of eating? [29:46]

Episode #147
In this Fasting Q&A episode, Megan Ramos answers some of your fasting questions:
1. Do you have any tips for getting better sleep while fasting? (Rose from Edinburgh) [01:57]
2. Is it possible to do a 10-day water fast without medical supervision? How long can one fast without needing medical supervision? (Maria from Toronto) [07:11]
3. When fasting for 36-72 hours, the Quick Guides say to eat lunch and dinner the following day. Is this a requirement or a suggestion as I find it easier to eat OMAD at dinnertime after work. (Mable from Montreal) [08:53]
4. My question is about fasting and constipation. How do I deal with it when skipping breakfast? (Lucilei from The Netherlands) [11:50]
5. I am doing 36-42 hour therapeutic fasting three times a week to reverse T2D. Once I have done that, what should my maintenance routine be? (Don from British Columbia) [15:44]
6. How do I balance staying adequately hydrated and not having to visit the washroom every one-two hours? I find it difficult to leave home as I constantly have to find the nearest washroom, but the worst is having to get up 3-4 times during the night and having interrupted sleep because of it. (Jane from Toronto) [18:09]

Tue, 26 Mar 2024 - 34min - 183 - Fasting Q&A with Dr Jason Fung: Ketones, Fat Adaptation, Carb Loading Before Surgery, Survival Genes, Dairy, and More
Episode #144
In this Fasting Q&A episode, hosted by Coach Lisa Chance, Dr. Jason Fung answers questions from the TFM Community:
- I’ve known for a long time how neuroprotective ketones are for the brain, but, wow, what ketones do for the heart is amazing, especially the failing heart. Could you elaborate, Dr. Fung? [02:15] Do you need to be in ketosis to burn fat and does being in at least a low level of nutritional ketosis help you go into autophagy sooner? [04:39] Why does it take such a long time to become fat adapted? [07:20] I’ve heard that fasting “turns on” survival genes. Can you tell us a little more about this? [09:25] What are your thoughts about the new recommendation to carb load before surgery to “calm” someone? [11:12] Is there any correlation between fasting and pulse rate? [12:58] I have a diabetic friend who is participating in a study on duodenal mucosa resurfacing (DMR) as a treatment for T2D. Does Dr. Fung have any take on this pretty new procedure or explanation of how and why it works? [13:17] If a person wants the benefit of therapeutic fasting (like autophagy) but they do not have adequate body fat for longer fasts (for example, a TOFI person with a fatty liver, or somebody at 30% body fat who needs a lot of autophagy for an autoimmune condition), How should they eat on eating days to be able to maintain a therapeutic fasting schedule like an alternate-day fasting protocol? [16:33] What are the signs of a histamine intolerance and can it get worse over time? Does fasting help? [17:53] If autophagy is mostly switched on by the presence of mTor, could a person (in principle) eat a zero protein diet and eat exclusively fat but still ramp up autophagy enough for some protein recycling to aid loose skin reduction, macular problems, skin tags, etc.? [18:45] How do you get rid of visceral fat? [20:22] How long does the effect of dairy on insulin last? [21:45] Why is it easier for women to do longer fasts after they’ve had a small amount of tubers during the last meal before their fast? [24:41]

Tue, 20 Feb 2024 - 33min - 178 - Fasting Q&A: Weight Loss and Excess Skin, Taking Medication, 16/8 Eating Windows, and More
Episode #139
In this Fasting Q&A episode, Nadia answers some of your fasting questions:
1. Which fasting protocol is best for getting rid of excess skin while you also get rid of excess weight? [01:58]
2. My GP has told me to take medication three times a day. How do I incorporate intermittent fasting into my lifestyle when I have to take meds three times a day? [09:26]
3. I’ve been fasting for two years. I lost 20 pounds the first year but gained 10 back in the second. I try to do 42-hour fasts but end up breaking it around the 20-hour mark. My blood glucose numbers are up and my weight isn’t coming down. I am a 45-year-old female. I am ready to give up. Please help. [13:50]
4. I was recently diagnosed with vulvar lichen sclerosus (LS). My gynaecologist says that it is believed to be an autoimmune disorder affecting the skin of the vulva. It is a horrifying condition that can cause changes to the architecture of the vulva, intense itching, tearing of the skin, etc. I’ve never known anyone to have this condition, but my gynaecologist says that she has a lot of patients with LS. Have any of your clients seen improvement in their LS? Is there a particular fasting protocol that you would recommend. I’m just devastated by this diagnosis. [21:43]
5. I understand that snacking (even with healthy foods) is a bad idea because it keeps your insulin levels up so how many meals would you suggest is the most effective during a typical 8-hour window in a 16/8 IF regime? Or maybe a better way to look at it is how many hours should I leave between meals to allow insulin to go back down? [26:55]

Tue, 06 Feb 2024 - 40min - 175 - Hot Topic: Therapeutic Fasting vs Fasting Sometimes
Episode #137

Episode #34
In this Fasting Q&A episode, Megan and Nadia answer your fasting questions:
1. I'd like to fast primarily for therapeutic reasons (mental health and autophagy especially) and aim to do three 42s. My issue is raging ketones! How high is too high when it comes to BK and at what point should I break a fast for this reason? [01:18]
2. What is the best protocol for weight loss? [11:58]
3. I am a TOFI and a pre-diabetic. What fasting can I do to reverse my T2D without losing weight? I noticed when I do extended fasting of 42 hours I lose too much weight. [21:18]
4. The one issue that I'm having the hardest time with is my mood during an extended fast (longer than 24 hours). I feel extremely irritable, agitated, quick tempered, & anxious, which is not how I normally am. I don't know if it's my cortisol levels, or what. Is this common? [31:08]

Tue, 28 Jun 2022 - 36min - 170 - Fasting Q&A: Chia Seeds, ADF for Weight Loss, Gut Flora, and Stress Hormones
Episode #32
In this Fasting Q&A episode, Megan and Nadia answer your fasting questions:
1. I know you have mentioned chia seeds (in water) as a way to gently break your fast, but can I use chia seeds as a training wheel during a fast because they give a feeling of fullness? [07:25]
2. I do 18:6 daily and a 48 each week. It took much effort to get my gut health stabilised. I have completed a couple of 5-day fasts in the past, but now I am worried about what a 5-day fast does to the good guys in my gut. With no fibre coming in, how do they survive? I don’t want to compromise my good gut flora, but I’d like the benefits of autophagy and stem cell generation. [13:44]
3. I’ve been ADF’ing for 6 weeks. I typically fast 36hrs, only consuming plain water, salt, and 2-3 cups of black coffee. I am 46 and have had seven pregnancies. I have hardly lost any weight, only a couple pounds. What am I doing wrong? On my eating days, I eat 3 meals over 12hrs, lower carb but not keto, and add 2 Ts cream to my coffee those mornings. I mostly do not snack. I find it very hard to fast 42hrs. Would three 42-hr fasts be more effective than ADF with 36hr fasts? Does it take a while for results when peri-menopausal? [18:42]
4. As someone “recovering” from PTSD, I have been taught skills and tricks to reduce cortisol and adrenaline. From a recent podcast I learnt that fasting increases these and I am now concerned that it may create effects counter to what I am trying to achieve to reduce PTSD symptoms. Have you had patients with a similar concern? Is there perhaps a difference in duration, or other helpful hormones released at the same time, to differentiate it from the perpetually elevated stress hormones after PTSD? [28:21]

Episode #30
In this Fasting Q&A episode, Megan and Nadia answer your fasting questions:
1. I am a nurse, and I work night shift (7pm-7am). I usually follow a 20/4 IF eating schedule on my days off, but I’m struggling to figure out how to fast on my night shift schedule when I work 2-3 nights in a row. Do you have any suggestions for fasting and a night shift schedule? [01:06]
2. Does coffee with heavy whipping cream break a fast? [08:38]
3. What and how do you eat when doing a "fat fast”? Do you still recommend following TRE? [15:51]
4. Most doctors will order fasting glucose and insulin, cortisol, and A1C to check for insulin resistance. Are there other markers (laboratory or clinical) to know if your IR is improving after, say, a year of fasting? [24:29]

Tue, 24 May 2022 - 48min - 165 - Fasting Q&A: eTRE, Weight-Loss Plateaus, Fasting and Menstrual Cycles, Dawn Phenomenon
Episode #27
In this Fasting Q&A episode, Megan and Nadia answer your fasting questions:
1. I’ve heard Dr. Fung say to skip breakfast, but recently in one of Dr. Nadia’s groups, I heard her recommend early time-restricted eating. Could you clarify this for me? [00:47]
2. This coming February I will have been fasting for two years. I fast 18 hours a day and exercise almost daily. I have hit the world's biggest plateau the last 6 months and just cannot lose any more inches/fat/weight. I'm definitely becoming more toned, but I'm not really getting smaller. I'm wondering if I'm at my body's resting weight or if I need to shake things up with my eating? I eat very balanced meals with protein, carbs and veggies. I could probably stand to NOT eat a cookie here or there. Since listening to your podcast, I have been sprinkling in one 24-hour fast a week and this week I am aiming for two 24-hour fasts. [08:26]
3. I've been fasting for a couple of months. I have read a lot about the fasting cycle with the menstrual cycle and I have heard you say it's easier to do longer fasts in the first week of the menstrual cycle. That's actually what I'm doing - three 36 or 42-hour fasts in my first week. The rest of my cycle I do 18-hour fasts and only 12-hour fasts in my 4th week. With that «protocol» I have lost 20 pounds in 3 months, but should I limit those longer fast to the first week of my cycle? [14:13]
4. How much does gluconeogenesis/the dawn phenomenon impact blood sugars early in the morning? A family member who is fasting and is type 2 diabetic has blood sugars of 150 or so at 6am but 95 later in the morning during a fast. I suspect the dawn phenomenon but I have no idea how much it could impact someone’s blood sugars. [20:20]

Tue, 17 May 2022 - 26min - 164 - Fasting Q&A: Calorie Deficit, Vegan Keto, Acronyms, Cooking Oils
Episode #26
In this Fasting Q&A episode, Megan and Nadia answer your fasting questions:
1. Does fasting for fat loss still require a caloric deficit? [01:08]
2. I've had success eating keto (traditional) with IF, and I've had success with a vegan diet (not keto) until I started relying too much on carbs like beans, rice, and potatoes. Do you think a vegan keto diet is a realistic way to eat while incorporating IF? [09:01]
3. I’ve heard a few acronyms that I don’t recognize: PCOS, CGM and IBS (although IBS is more common and not specific to fasting). For those used to fasting and the TFM Community discussions, these are common terms. However, I believe that those not versed would be left wondering about them and possibly looking them up. I know I had to look up PCOS when I first heard it. [13:56]
4. I’m confused about which cooking oils are inflammatory and why. Can you please address this? [20:48]

Tue, 10 May 2022 - 28min - 163 - Fasting Q&A: Maintenance, Metformin, Optimizing Fasting Days, Weight Gain While Fasting
Episode #25
In this Fasting Q&A episode, Megan and Nadia answer your fasting questions:
1. I have been fasting on and off for years and had moments of great weight-loss success, but it seems like as soon as I stop I put the weight back on, regardless of how well I'm eating. Am I just fated to have to continue fasting forever to keep my weight down? [00:40]
2. My doctor wants me to start taking metformin, but I would like to incorporate fasting to bring down my A1C which is currently 7.1%. What are the benefits (if any) of using metformin for someone who is just starting on their fasting journey? Can you even take metformin on fasting days? [06:24]
3. Every fast is different. Why is that? I have been experiencing
euphoria and loads of energy while fasting, and fasting has been
wonderful. I have also been down, tired, and with no energy while
fasting. I'm thinking that the answer could have something to do with
water and electrolytes. What can I do to optimize the fast
and feel euphoric more often? [16:10]
4. Have you ever had someone gain weight while fasting? Are there common mistakes that people make if this is the case? [22:01]

Episode #23
In this Fasting Q&A episode, Megan and Nadia answer your fasting questions:
1. You say not to nurse coffee that contains cream. Is it OK to nurse a cup of black coffee or hot tea (without caffeine in it) during your fast? [02:20]
2. During the week before menstruation should I not fast at all? I would love a guide that gives a general idea of what types of fasting are recommended related to where a woman is in her cycle, keeping hormones in mind? [10:47]
3. I decided to do time-restricted eating for life and it's working for me, but I don't understand why it's working for me? [20:03]
4. Is resistance training in a fasted state, specifically longer fasts of 72 hours, a waste of time when there’s no intake of protein before and after sessions to trigger muscle protein synthesis? [27:55]

Tue, 05 Apr 2022 - 46min - 158 - Fasting Q&A: Post-Menopausal Bleeding, Loose Stools After Breaking Fasts, No Gallbladder and Keto, Hashimoto’s and Fasting
Episode #20
In this Fasting Q&A episode, Megan and Nadia answer your fasting questions:
1. I’ve been in menopause for over two and a half years and doing IF for the past year. I have done a couple of extended fasts. I belong to a women’s fasting group on Facebook and have read that a couple of women in menopause have started menstruating again and, weirdly enough, that just happened to me. Is this because of IF and have you seen this happen? Please tell me it does not mean I have a chance of getting pregnant! [01:22]
2. I get diarrhea about an hour after eating on OMAD. I have tried to eat a small amount and wait for a half hour, or longer, before eating again. I try to chew thoroughly and slowly, but nothing seems to help. [04:57]
3. Is keto and a higher fat intake (even though its healthy fats) without a gallbladder harder on my pancreas to process fats or should I maybe try a Mediterranean diet and fasting. I have a lot of weight to lose. [09:53]
4. Does fasting do more harm than good for people with Hashimoto's? [17:26]

Tue, 29 Mar 2022 - 25min - 157 - Expert Guest: Dr. Jason Fung
Episode #19
In today's episode, Megan and Nadia FINALLY interview the co-founder of The Fasting Method Program, Dr. Jason Fung.
Dr. Fung talks about the benefits of health coaching and highlights the connection between diet and emotions. Hear Jason's take on how our habits have changed over the last few decades, influenced by external factors and how this "new environment sets us up to fail” and not that our "will-power" is now weaker. Learn from Dr. Fung how "obesity spreads like a virus" and how community is so much more important than even diet and fasting for success: "Trying to do it yourself is impossible" ..."it takes a village".
Jason, Megan and Nadia talk about the TFM community and the power of peer support vs. the "top-down" medical approach and why the Community members have been so successful in maintaining their healing and weight loss over the years.

Episode #18
In this Fasting Q&A episode, Megan and Nadia answer your fasting questions:
1. When you refer to lowering insulin level, is that the same as lowering blood sugar level? If it is not the same, how do we know our insulin level is lowering? [08:36]
2. Do you know if intermittent fasting plays a role in improving endothelial function? More specifically, helping with erectile dysfunction? [16:29]
3. I was just wondering what were your thoughts on working out during a fast? I have read other sources stating that working out after hour 16 (when fat burning is ramping up) would help to burn more fat? [22:32]
4. Would three 42-hour fasts be more effective than ADF with 36-hour fasts? Does it take a while for results if in peri-menopausal? [34:06]

Tue, 18 Jan 2022 - 29min - 147 - Fasting Q&A: Flavored Coffee, Feeling Cold, Menstruation, and Switching Up Your Fasts
Episode #9
In this Fasting Q&A episode, Megan and Nadia answer your fasting questions:
1. I find that drinking black regular/unflavoured coffee is hard (on my stomach as well as well as on my stamina!!!) but I find it easier to drink black flavored coffee so that I don't have to add milk to it which would be my preference. By flavored, I mean the beans are flavoured (the company says that even their flavored coffees have only 5 calories in a cup of coffee). I do not add flavored syrups. Is it OK to drink flavored coffee or does it break my fast? [01:36]
2. Fasting during menstrual cycle? [22:45]
3. Feeling so cold during a fast that it stops you continuing. [14:18]
4. Since I’m not losing weight on the 16:8 fasts after two months, I want to start 24 hours of fasting. What will happen in, let’s say a year, when I hopefully lose weight and get off some meds and go back to just 16:8. Will I regain the weight and will 24 hours of fasting be a lifetime thing? [33:49]

Tue, 14 Dec 2021 - 51min - 143 - Fasting Q&A: Ending an Extended Fast, OMAD, Keto and Low-Carb, and More
Episode #005
In this Fasting Q&A episode, Megan and Nadia answer your fasting questions:
1. When should you end an extended fast? [00:30]
2. I am starting a 30:16 cycle and hope to move onto a 3x42. I have about 10 pounds more to lose. I feel very satisfied with one meal a day and 800 calories, but I am concerned that I might damage my metabolism if I go very long on OMAD (one meal a day). Is that something that is OK or should I make sure I get more calories 3 or 4 days a week? [09:08]
3. When doing one-meal-a-day, how much should you eat and/or how long should you eat? [17:50]
4. When it is said, “Intermittent fasting is not about diet,” why are we told to do keto or low-carbohydrate diets? [24:56]

In today's episode, we share our journey from illness to being able to help the people we work with lose weight and reverse the chronic and "incurable" type 2 diabetes.
Truth be told, we started this podcast because there is so much misinformation out there about intermittent fasting. Today our team at The Fasting Method has worked with over 16,000 adults worldwide helping them achieve sustainable weight loss and improve blood glucose levels through various fasting strategies and real, whole-food nutrition principles. We believe we have a responsibility to bust these fasting myths and set the record straight.
And we're not just practioners of fasting; it's saved our lives. We've struggled with obesity, type 2 diabetes, polycystic ovarian syndrome, and fatty liver disease. We were able to take the lessons we learned throughout our own journeys and apply them, first to our clincial practice and then to our online community, and now we want to bring them to you in this podcast.

Tue, 16 Jan 2024 - 34min - 135 - Fasting Q&A: IBS, Raised Cholesterol, Fat-Loss Maintenance, Busting Plateaus, and Erythritol
Episode #134
In this Fasting Q&A episode, Nadia answers some of your fasting questions:
1. I have IBS-C and a gluten sensitivity, along with ever changing hormones as I am perimenopausal. I’m trying to find the best regimen to help. Of course weight loss would be nice but my main goal is to rid myself of abdominal pains, severe constipation, and nausea. [01:43]
2. My mom has type 2 diabetes and is taking Metformin. I have convinced her to do at least 16 hours of fasting a day and she has been doing this for 5 months or so. She says her glucose levels are much more stable but her cholesterol has gone up. Should she worry? [09:21]
3. Once you have reached your fat-loss goal, how do you transition to a maintenance schedule without gaining back the weight? And what would a good fat-loss maintenance schedule be? [15:10]
4. I started fasting about a year and a half ago with a very mixed schedule and was successful in losing about 25 lbs in 8 months. Since then I’ve been at a plateau for another 8 months. I find it hard to not eat at least once per day. What are your thoughts on ADF with the 500 cal vs without the 500 cals? [17:35]
5. I don’t think I’ve heard you discuss the sweetener erythritol. I realize that it may still not be great for sugar cravings, but does it affect insulin? If I had a small amount in my tea, would it break my fast? I’ve seen conflicting messages? [25:16]

Fri, 15 Dec 2023 - 10min - 128 - Fasting Q&A: Food Quality, Plant-Based Diets, and Fiber
Episode #127
In this Fasting Q&A episode, Nadia answers some of your fasting questions:
1. What are your thoughts on food quality? Should I try and eat grass-fed meats and organic vegetables? How important is it to switch to this top-quality food alongside fasting? [02:21]
2. Is bacon a processed meat? Is having non grass-fed bacon ok because, here in India, we can’t get grass-fed bacon? [05:41]
3. Do you have any tips for fasting on a vegetarian diet? My partner is a vegetarian and I am interested in cutting out meat also, but I am not sure about the nutritional value of meat substitutes like tofu and other plant-based proteins. [07:28]
4. I would love to hear more about fasting with a plant-based diet. I don't avoid fruit or below-ground veggies. I eat nuts and seeds, but no dairy. I have cut out most highly processed foods and I am doing TRE, usually a 16-18 hour fast. There is so much conflicting information about diet! [09:57]
5. I follow a high-fat, low-carb diet on my eating days. I seem to struggle to keep my carbs low while trying to get in my daily fiber. I try to eat as much high-fiber food as possible (like avocados, chia seeds, and vegetables) at each meal. How do you add fiber to your diet while staying low-carb? Should I consider fiber supplements? [13:59]
6. Being that fiber helps to slow the digestion of sugars and fats (therefore decreasing blood glucose spikes), would you recommend taking a daily fiber supplement? What about when fasting? Does it technically break a fast even though the body is not digesting it? [18:15]

Fri, 24 Nov 2023 - 10min - 122 - Fasting Q&A: Feeling Cold, Night Sweats, Gestational Diabetes, and Eating During Pregnancy
Episode #121
In this Fasting Q&A episode, Nadia answers some of your fasting questions:
1. I used to do 16-8 IF and have now started doing ADF. How can I avoid malnutrition doing this new pattern? [01:45]
2. I am planning on doing 24-hour fasts three days a week for fat loss. For this schedule, you typically recommend (on eating days) to still fast 16 hours and only eat two meals. But, on my eating days, would it be better to not fast at all and eat three meals to keep up my metabolism? If not, how do I maintain my metabolism during this process? [08:08]
3. I am doing three 42-hour fasts a week. On my fasting days, every time I reach 24 hours into my fast, I start feeling cold. What is the process behind that reaction?
I’ve also found that since I started fasting eight weeks ago, my perimenopause symptoms have increased, namely I experience more and more severe night sweats. Is this just a coincidence or could fasting be increasing these symptoms. [14:06]
4. I started my fasting journey after listening to your podcast, which I love. I'm now up to 24 hours three times a week and looking forward to doing longer ones soon!
My question for you is about gestational diabetes. I’ve had it with both of my kids and was told that eating healthy has nothing to do with getting it. I’m curious what you think about this. I’m determined to get healthy through fasting to prevent T2D in the future and am hoping to prevent gestational diabetes as well as pre eclampsia (I had hypertension with my first and pre eclampsia with my second!) Is there a way to prepare my body to prevent either of these with my next pregnancy. [20:41]
5. Do pregnant women have to eat more than three good meals a day? I get full so quickly and I’m not sure I can eat more than three meals a day. I normally ate two meals a day when I fasted before pregnancy. [25:05]
